What do I need to bring with me for my first visit?

For your initial visit, remember to download, fill out and bring in your new-patient information forms, which you can download here. Also please bring your insurance information, any relevant medical records, X-rays or other medical imaging, and a list of the medications you take. Will my pain care involve surgery?

Knowing what your treatment plan will consist of cannot be known in advance of an exam, medical history, physician consultation and definitive diagnosis. However, Cedar Rapids Pain Associates pain management physicians provide nonsurgical care, including minimally invasive procedures that can relieve pain with minimal impact on the body. Yet, as a multidisciplinary team committed to your best health and most complete relief, we may recommend surgery if — and only if — it is your best option.

Do you normally take tests at the first visit? What kind?

As part of our dedication to efficient, high-quality care, we generally strive to be as efficient as possible. This means we may perform some tests during your first visit, depending on a number of factors. It also means that we need to see your prior imaging and test results. Our pain management specialists may order additional tests, if necessary.

Will I get your treatment recommendations during my first visit, or will I have to wait for test results to learn what choices I may have for treatment?

Cedar Rapids Pain Associates’ providers will make treatment recommendations only after careful evaluation and a thorough understanding of the problem and surrounding circumstances. Prior to having all results, we may know enough to be able to predict the types of treatments that could be appropriate and effective. But we will only recommend treatments once we have the answers we need to help you make an informed decision. In many cases, though, we do have enough information to be able to recommend specific treatment options during the first visit.

Will you work with my primary-care physician to make sure I get what I need?

At Cedar Rapids Pain Associates, as specialists dedicated to your relief and best quality of life, our mission is to support your doctor in the delivery of the best possible care. We take pride in cultivating great relationships with referring doctors, starting with making referrals easy, providing regular reports, and always being accessible and responsive to consult with your physician.

Will my insurance plan cover my treatment and any tests you order for me?

That depends entirely on your insurance plan… but we’ll be happy to help with that. When you call for an appointment, we’ll do an insurance pre-authorization, and we can also help determine what is or isn’t covered, including tests and treatments. Generally, insurance covers tests and treatments that are medically necessary. The extent of coverage and patient financial responsibility varies from plan to plan, and should be explained in your Summary of Plan Benefits. You may also wish to contact your insurance carrier directly.

What insurance plans do you accept?

Do you file my insurance claim for me?

Yes, our friendly, helpful, caring staff will file your claim on your behalf, whether we are in your insurer’s network or filing on an out-of-network basis.

Where will my treatment be performed?

We perform all minimally invasive treatments, therapies and other care in the comfort and convenience of our office.
